2021-2022 REGISTRATION INFORMATION
Saturday, July 31, 2021 (7:00 a.m. – 7:00 p.m.)
Registration Forms are available to print on school’s website
www.lostantcomets.org
Two (2) proofs of residency are needed at Registration. The proof of residency can be a:
· Gas bill
· Water bill
· Electric bill
· Telephone bill
· Tax bill
· Mortgage coupon
· Rental Lease agreement
Students in the grades of Kindergarten, 2nd grade, 6th grade, and any student playing a sport will need to have the following information completed:
Kindergarten:
· Physical Exam (prior to the 1st day of school)
· Dental Exam (due by May 15th)
· Eye Exam (by an optometrist or ophthalmologist within a year)
· Lead Screening (at doctor’s appointment)
· Home Language Survey
· Certified Copy of Birth Certificate
· Up to date immunizations
2nd Grade:
· Dental Exam
6th Grade:
· School Physical
· Up to date immunizations
· Dental Exam
Students playing a sport:
A sports physical must be submitted to the office.
